The Century College Behavioral Interventions and Threat Assessment team works with students to find solutions to academic, social, and personal situations that may impact the potential for student success.

Use this report if you observe incidents of concerning, unusual, dangerous, or threatening behaviors.

Conduct Violation

This report is for faculty use only. Faculty use this report to submit student academic honesty Code of Conduct violations, such as plagiarism, cheating, or collusion.

Academic Honesty Report

Use this form to file a complaint based on sexual misconduct. Reports may be made by:

• An individual who has experienced sexual misconduct.
• Anyone who receives a report from someone who experienced sexual misconduct.
• Anyone who witnesses or otherwise has information that sexual misconduct may have occurred.

Reports can be submitted anonymously.

A bias incident is an act of bigotry, harassment, or intimidation that is motivated in whole or in part by bias based on an individual’s or group’s actual or perceived race, color, creed, religion, national origin, gender, age, marital status, disability, public assistance status, veteran status, sexual orientation, gender identity, or gender expression.
