REQUIRED INFORMATION
Prepare for a safe, productive visit to the Century College Dental Hygiene Clinic! Review this list of what to bring to your appointment and what safety information to gather if you have a pre-existing health condition.
- A list of the prescriptions, over-the-counter medications and herbal supplements that you are taking, including dosages.
- The name of your physician and/or medical clinic.
- The name of your dentist and/or dental clinic.
- A list of any questions or concerns to discuss prior to your treatment.
- Any information required if you have a pre-existing condition as listed.

If you have a history of any of the following conditions, ask your doctor to check if you need to take antibiotic medication before dental treatment. If you need pre-medication but don’t take it before your appointment, we will reschedule your visit.
Conditions that require pre-medication include a history of:
- Infective endocarditis
- Heart valve replacement, transplant and certain congenital heart conditions
- Other conditions or procedures for which your doctor or surgeon requires pre-medication.
Clotting disorders: Patients that are taking blood thinner medications that require INR checks, should have their INR evaluated prior to their dental appointment and have the information available upon request.
Diabetes: Patients with diabetes should bring their most recent HbA1c and glucose reading to the appointment.
