Access Center Documentation

• Individualized Education Plan (IEP) or high school special education evaluation  
• Section 504 Plan 
• Accommodation Plan from another college or university 
• Military service-connected disability documentation 
• Neuropsychological or psychological evaluations 
• Letter from a provider (e.g., therapist, doctor, etc.)  

We use documentation to help understand your needs and create a plan that works well for you. Documentation that includes the following information assists us in determining an accommodation plan for your needs:  

• The current disability and the history of diagnosis  
• Anticipated prognosis (if applicable) 
• Specific symptoms, including frequency and severity 
• The extent to which the disability impacts major life activities (such as communicating, reading, writing, learning, working, walking, eating, breathing, etc.) 
• Recommendations that would reduce the impact of the disability

• Be typed or printed on official letterhead and be signed by an evaluator qualified to make the diagnosis, or with the signature of the treating qualified provider 
• Reflect the current impact of your disability.  For example, we may request additional documentation from students who had an IEP or 504 Plan if these plans were not in effect immediately before the date when they left high school.
  

• Upload it to your AIM portal when completing the Intake Application
